select “CAPITOLA” from the “Which office would you like to visit?(Make a selection from the map or the drop down list of offices.)” listbox
turn on the “1 item” radiobutton
turn on the “Register or title a vehicle or vessel (boat):” checkbox
enter your “First Name” into the first “First Name:” textbox
enter “cypher” into the first “Last Name:” textbox
enter your “home area code” into the “Telephone #:” textbox
