- click the “submit” button
- turn on the “Permanent Opt-Out by Mail:” radiobutton
- click the “Submit” button
- enter your “First Name” into the “First Name” textbox
- enter your “Last Name” into the “Last Name” textbox
- YOU enter your SSN and DOB
- enter your “home street address” into the “Street Address:” textbox
- enter your “home city” into the “City:” textbox
- select your “home state abbrev” from the “State:” listbox
- enter your “home zip code” into the “Zip:” textbox
- enter your “home phone with dashes” into the “Telephone” textbox
- YOU type the characters shown
- click the “Confirm” button
- click the “click here to print” link
