click the “Create A Hold Mail Request” button
enter your “first name” into the “First Name:” textbox
enter your “last name” into the “Last Name:” textbox
enter your “street address” into the “Address:” textbox
enter your “phone number” into the “Phone:” textbox
click the “Continue” button
click the “Continue” button
select your start date from the “Begin Date:” listbox
